1. Our office and staff are available from 8:30am to 5pm Mon-Fri.
2. We have a physician on call for emergencies 24 hrs/day 7 days/week
3. We do NOT refill any controlled substances after hours and on weekend. Other medications may be called in on a case by case basis until the patient can see their primary doctor.
4. A complete copy of your medical record is available to you for a cost of $25.
5. Your insurance Co-pay is expected at the time of service. If there is no insurance or if we are not on your insurance plan, payment for the office visit is expected at the time of service.
